Code of Practice

Initial concern is expressed by teacher, parent, health or social services professional
Teacher informs Headteacher and SEN coordinator. SEN coordinator registers child's special educational needs
Teacher/SEN coordinator gathers information, makes an initial assessment and informs Head of Learning Support (Primary)
SEN coordinator seeks any further necessary information/administers relevant assessments to identify specific needs of child
A Child Review is held with parent, Headteacher, teacher, SEN coordinator and HOLS(P)
No special education help needed, but keep under review Give special help within the normal classroom setting using the support of General SEN Support Assistant, keeping record of nature and aims of such provision 
When no longer any cause for concern, child removed from SEN register and parents informed  Devise Individual Education Plans for any pupils with severe learning difficulties. Education Support Assistant to implement individual education plan under the guidance and direction of the class teacher
